Medical Insurance
We are providers for:

We will file claims for other private insurance plans. Our office always recommends that you check with your insurance
company if you have any questions regarding your coverage.
Please note: A refraction is a measurement of the lens power necessary to prescribe glasses or other corrective
lenses. Medicare as well as some other medical plans do not cover refractions (or routine eye examinations when no medical
problem is known or suspected). Medicare/other medical plans allow us to charge separately for the refraction portion of the
examination. If your insurance company does not pay this service, you are responsible for this charge.
If you have a separate vision plan that covers routine or annual eye examinations, please let us know. Your vision plan
may assist you with your eye care needs that are not covered by your medical plan.
Billing policy
New patients with no insurance are required to pay the day of service. Co-pays must be collected at the time of service.
Any balance remaining after insurance pays is due within 30 days of receipt of statement. LASIK fees are due 1 week prior
to the day of your treatment.
